Burns
Second- and third-degree burns treated within 72 hours of accident

Emergency Room Treatment
Within 72 hours of accident
Hospital Admission
Within 180 days of accident - not included: treatment in emergency room, outpatient
treatment, a stay of less than 20 hours
Hospital ICU Confinement
Within 30 days of accident - if patient is in ICU for more than 15 days, the hospital
confinement benefit begins on 16th day
$200 Per day for up to 15 days - any insured

Wellness
Annual Physical, Dental Exam, Mammogram, Pap Smear, Eye Examination, Immunization,
Flexible Sigmoidoscopies, PSA Test, Ultrasounds, Blood Screening Test
$60 per year. Policy must be in force 9 months before this benefit is payable
